Determining the correct maxillomandibular relationship is crucial for prosthodontics. Removing occlusal interferences significantly minimizes posterior displacement of the mandible during retrusion.

Background:

  • Debate exists regarding the optimal anteroposterior maxillomandibular relationship for fixed and removable prosthodontics.
  • This is particularly critical in complete denture fabrication where natural occlusal guidance is absent.

Purpose of the Study:

  • To quantify posterior displacement at the mandibular condyles and occlusal surfaces.
  • To measure displacement when the mandible moves from maximal intercuspation to the retruded mandibular position.

Main Methods:

  • Articulated occlusal models from 18 subjects with natural dentition were analyzed.
  • Models were rearticulated in retruded contact position, and occlusal adjustments restored vertical dimension.
  • Condylar position and occlusal surface displacement were measured using specialized articulators and microscopes.

Main Results:

  • Initial condylar retrusion ranged from 0.6 to 2.4 mm, reduced to 0-0.4 mm after occlusal adjustment.
  • Occlusal surface retrusion ranged from 0.4 to 1.5 mm, reduced to 0-0.5 mm post-adjustment.
  • Mean condylar retrusion decreased from 1.0 mm to 0.2 mm; mean occlusal retrusion decreased from 0.7 mm to 0.2 mm.

Conclusions:

  • Posterior displacement at both condyles and occlusal surfaces is minimal.
  • Removing interfering occlusal contacts on the retruded path is key to achieving this reduction.
