    Summary

    Intrauterine fetal vesicoureteral reflux was observed in a fetus and persisted after birth. This suggests fetal cystograms may reveal a higher reflux incidence than previously thought.

    Background:

    • Vesicoureteral reflux (VUR) is a condition where urine flows backward from the bladder to the ureters.
    • Intrauterine diagnosis of VUR is challenging, with limited data on its incidence and natural history.
    • Fetal transfusions offer a unique window for in-utero urinary tract assessments.

    Purpose of the Study:

    • To document intrauterine fetal vesicoureteral reflux (VUR) in a 25-week fetus.
    • To assess the persistence of VUR in infancy via follow-up cystourethrograms.
    • To evaluate the incidence of VUR in fetal cystograms compared to postnatal data.

    Main Methods:

    • Performed a fetal cystogram at 25 weeks gestation.
    • Obtained a voiding cystourethrogram (VCUG) in the patient at 1 year of age.
    • Surveyed physicians performing fetal transfusions regarding their experience with fetal cystograms and VUR.

    Main Results:

    • Bilateral VUR was identified in the fetus at 25 weeks gestation.
    • The VUR persisted at 1 year of age, confirmed by VCUG.
    • No urinary tract infections were documented in the patient.
    • Physicians reported infrequent fetal cystogram use and observed VUR in a small subset of cases.

    • This diagnostic approach allows for monitoring VUR progression or resolution in affected children.
