Background:

  • Appendiceal carcinoma is an exceptionally rare malignancy.
  • Preoperative diagnosis of appendiceal neoplasms is notoriously difficult, even during surgical exploration.

Observation:

  • A patient presented with a suspected right ovarian adnexal tumor, confirmed by ultrasound.
  • Laparotomy revealed an appendiceal tumor, a concurrent small bowel tumor, and enlarged lymphatic nodules.

Findings:

  • Surgical resection included the uterus, adnexa, appendix tumor, segments of small bowel and colon, and part of the bladder due to infiltration.
  • The patient received 5-fluorouracil (5-FU) chemotherapy post-operatively.

Implications:

  • This case underscores the diagnostic challenges of non-ovarian pelvic masses.
  • Laparotomy remains a critical tool for definitive diagnosis and management of such rare conditions.
