Summary

Chronic obstructive pulmonary disease (COPD) is rarely found in patients evaluated for pulmonary hypertension. However, when present, severe pulmonary hypertension can be associated with COPD, even in non-smokers.

Background:

  • Pulmonary hypertension (PH) is often linked with chronic obstructive pulmonary disease (COPD).
  • Previous research focused on COPD patients diagnosed with PH.
  • The prevalence of COPD in individuals referred for PH evaluation is not well-documented.

Purpose of the Study:

  • To investigate the occurrence of COPD in patients referred to a pulmonary hypertension clinic.
  • To characterize the severity of PH in patients with co-existing COPD.

Main Methods:

  • Retrospective chart review of approximately 600 patients at a tertiary care PH clinic.
  • Inclusion criteria: age >40, no secondary causes of PH, evidence of COPD via PFTs or CT scan.
  • Data collected included pulmonary artery pressures and pulmonary vascular resistance.

  • Five patients met the criteria (3 males, 2 females, mean age 65).
  • All patients were former smokers.
  • Average mean pulmonary artery pressure (MPAP) was 59 mmHg; mean pulmonary vascular resistance (PVR) was 15.2 mmHg/L/min.
  • Only one patient showed vasodilator response to adenosine.

Conclusions:

  • COPD is uncommon in patients primarily evaluated for pulmonary hypertension.
  • Severe pulmonary hypertension can infrequently be associated with COPD.
