    Background:

    • Megaureter-megacystis is a complex urinary tract condition characterized by massive primary vesicoureteric reflux and its sequelae in pediatric patients.
    • Understanding the typical presentation and potential diagnostic challenges is essential for appropriate management.

    Observation:

    • This study reviewed 22 male children diagnosed with megaureter-megacystis.
    • Initial symptoms, radiographic findings, and diagnostic processes were analyzed.
    • Diagnostic misconceptions were a significant issue, leading to delayed or incorrect interventions.

    Findings:

    • 16 out of 22 boys received incorrect diagnoses, including obstruction, neurogenic bladder dysfunction, and isolated megacystis.
    • Inappropriate treatment was administered to 14 children.
    • Only 8 boys experienced prompt and accurate diagnosis and treatment.

    Implications:

    • Misdiagnosis of megaureter-megacystis can lead to suboptimal patient outcomes.
    • Improved diagnostic criteria and physician education are needed to enhance early and correct identification.
    • Timely and appropriate management is critical for preventing long-term sequelae in affected children.
