Summary

Nocardia asteroides caused unusual lymphocutaneous infections in two healthy individuals after plant punctures. Prompt treatment with trimethoprim-sulfamethoxazole and debridement led to successful recovery.

Background:

  • Nocardia species are ubiquitous in soil and organic matter.
  • Lymphocutaneous nocardiosis, or nodular lymphangitis, is an uncommon presentation.
  • Typically acquired via traumatic skin inoculation, often by Nocardia brasiliensis in immunocompetent hosts.

Observation:

  • Two immunocompetent patients presented with nodular lymphangitis after gardening injuries involving plant thorns.
  • Clinical manifestations included ulcerative lesions and abscessed inflammatory nodules along the lymphatic tract.
  • No systemic involvement was observed in either case.

Findings:

  • Microbiological analysis of purulent drainage and skin biopsies confirmed Nocardia asteroides as the causative agent.
