Summary

Postoperative radiotherapy is recommended for patients with residual nonfunctioning pituitary macroadenoma after surgery. Complete tumor removal minimizes recurrence risk, making routine radiotherapy unnecessary in such cases.

Area of Science:

  • Neurosurgery
  • Endocrinology
  • Radiation Oncology

Background:

  • Nonfunctioning pituitary macroadenomas are common intracranial tumors.
  • Surgical resection is the primary treatment, but recurrence is a concern.
  • The role of adjuvant radiotherapy remains debated.

Purpose of the Study:

  • To identify indications for postoperative radiotherapy after surgical resection of nonfunctioning pituitary macroadenoma.
  • To evaluate the efficacy of radiotherapy in preventing tumor recurrence.
  • To determine prognostic factors for recurrence.

Main Methods:

  • Retrospective chart review of 72 patients with chromophobe adenoma.
  • Minimum follow-up of 12 months, with a mean follow-up of 64 months.

Related Experiment Videos

  • Proportional hazards model used to identify prognostic factors.
  • Main Results:

    • Tumor recurrence was significantly lower in patients who received postoperative radiotherapy (26%) compared to those who did not (46%).
    • In patients with residual tumor, radiotherapy reduced recurrence rates from 73% to 32%.
    • Complete tumor excision was associated with a low recurrence rate (10%), even without radiotherapy.

    Conclusions:

    • Complete surgical excision should be the primary goal.
    • Routine postoperative radiotherapy can be withheld in patients with no residual tumor.
    • Postoperative radiotherapy is effective in reducing recurrence in patients with residual tumor.
