Pleural fluid cholesterol levels correlate with serum cholesterol and pleural permeability. Lipoproteins in pleural fluid undergo changes, and triglycerides show weak correlation, impacting transudate/exudate differentiation.

Background:

  • Differentiating pleural fluid (PF) transudates and exudates is crucial.
  • While PF cholesterol testing has been proposed, factors influencing its levels remain unclear.

Purpose of the Study:

  • To investigate the relationships between serum (S) and PF levels of total cholesterol (CHOL), LDL cholesterol, HDL cholesterol, and triglycerides (TRIG).

Main Methods:

  • Analyzed S and PF from 99 patients (13 transudates, 86 exudates) for CHOL, HDL, LDL, TRIG, apolipoproteins, and protein.
  • Used linear and multiple regression to assess relationships between S and PF levels, incorporating the PF/S protein ratio.

Main Results:

  • PF cholesterol levels strongly correlate with S cholesterol and pleural permeability (r=0.88, p<0.001).
