Background:

  • Chronic granulomatous disease (CGD) involvement of the gut, particularly colitis, is recognized but poorly characterized.
  • Previous understanding suggested CGD colitis was non-specific or mimicked Crohn's disease.

Observation:

  • Seven pediatric patients with CGD and gastrointestinal symptoms were prospectively studied.
  • Clinical presentations included anemia, failure to thrive, and diarrhea.
  • Endoscopic findings revealed friable, erythematous mucosa in most patients.

Findings:

  • Histopathology demonstrated a unique colitis characterized by neutrophil paucity, eosinophilia, eosinophilic crypt abscesses, pigmented macrophages, and nuclear debris.
  • Granulomas were present in a minority of cases.
