Summary

Patients with limited testicular microlithiasis (TM) have a lower risk of malignancy than classic TM. The short-term risk of developing testicular cancer in isolated TM is low, questioning the need for routine surveillance.

Background:

  • Testicular microlithiasis (TM) is an ultrasound finding with uncertain clinical significance.
  • Distinguishing between classic TM (CTM) and limited TM (LTM) may impact risk stratification.

Purpose of the Study:

  • To evaluate the short-term natural history of CTM and LTM.
  • To assess the association between TM subtypes and testicular tumor development.

Main Methods:

  • 104 patients with TM identified via ultrasound were classified into CTM (≥5 microliths) or LTM (<5 microliths).
  • Patients underwent baseline and follow-up ultrasounds to monitor TM changes and tumor development.

Main Results:

  • Malignancy was present at diagnosis in 18% of CTM patients versus 2% of LTM patients (P=.004).
