Background:

  • Dermatofibromas with granular cells have been documented.
  • This report details a granular cell tumor presenting with dermatofibroma architecture.

Observation:

  • A 2.5-cm oval, hyperpigmented plaque was observed on a 60-year-old African-American woman's back.
  • The lesion had been present for years.

Findings:

  • Histopathology revealed granular cells with spindle shapes, resembling dermatofibroma.
  • Immunohistochemistry showed tumor cells positive for S-100 protein and neuron-specific enolase, but negative for factor XIIIa.

Implications:

  • This case represents the first reported histological variant of a granular cell tumor.
  • Immunohistochemical results support the diagnosis of a granular cell tumor with a dermatofibroma-like pattern.
