Background:

  • Thymic cysts are rare congenital or acquired lesions.
  • Thymoma is a common primary tumor of the anterior mediastinum.

Observation:

  • A 77-year-old woman presented with an asymptomatic 3.5 cm cystic mass in the left anterior mediastinum on CT scan.
  • Surgical resection via total thymectomy was performed.

Findings:

  • Histological examination revealed a thymic cyst lined by cuboidal cells.
  • A markedly thickened area of the cyst wall showed features of thymoma.
  • This suggests thymoma originated from the thymic cyst wall.

Implications:

  • This case highlights a potential, albeit rare, pathway for thymoma development.
