Summary

This rare CD2- CD4+ CD56+ lymphoid malignancy primarily affects the skin and has an aggressive course. Despite treatment, this cancer shows a poor prognosis and rapid relapse, highlighting the need for further research.

Background:

  • CD2- CD4+ CD56+ lymphoid malignancy is a rare hematologic disorder.
  • It is characterized by frequent skin involvement, agranular cells, and aggressive progression.
  • Epstein-Barr virus (EBV) is typically not involved in this condition.

Observation:

  • A unique case of a Japanese patient with a CD4+ CD43+ CD56+ CD2- phenotype is presented.
  • The malignancy involved skin, nasopharynx, bone marrow, and lymph nodes, presenting as purplish nodules.
  • Histopathology revealed atypical pleomorphic cells in the dermis; no T-cell receptor (TCR) or immunoglobulin gene rearrangement was detected, nor was EBV identified.

Findings:

  • The study identified 17 similar cases from literature, all sharing CD2- CD4+ CD56+ and germline TCR rearrangement.
