Community-level indicators of chronic health conditions and services

Programs targeted toward chronic conditions provide a stimulus for development of program performance indicators. This paper illustrates how existing information can be organized at community and health plan levels to produce these indicators. Data compiled by claims and encounter systems are emphasized. Survey data can be incorporated in the indicator system on a targeted basis, particularly for information on community services and linkages to healthcare and also for caregiver perspectives.
