Summary

Primary lymph node plasmacytoma (PLNP) may be a distinct plasma cell neoplasia. PLNPs showed unique immunophenotypes and did not progress to multiple myeloma, unlike other extramedullary plasmacytomas.

Area of Science:

  • Hematology
  • Oncology
  • Pathology

Background:

  • Plasma cell neoplasms encompass a spectrum of disorders, including multiple myeloma and extramedullary plasmacytomas.
  • Primary lymph node plasmacytoma (PLNP) is a rare entity whose distinctness among plasma cell dyscrasias requires further investigation.

Purpose of the Study:

  • To ascertain if primary lymph node plasmacytoma (PLNP) represents a distinct entity within plasma cell neoplasms.
  • To compare the clinicopathologic features of PLNPs with non-nodal extramedullary plasmacytomas and multiple myeloma.

Main Methods:

  • Analysis of a large series of PLNPs from two major lymphoma registries.
  • Comparative study of histologic, immunophenotypic, and clinical data from 25 PLNPs, 10 non-lymph node plasmacytomas, and 51 multiple myeloma cases.

Related Experiment Videos

  • Inclusion of 36 reactive plasmacytoses as control cases.
  • Main Results:

    • Histologic features of PLNP were similar to other extramedullary plasmacytomas, showing immaturity compared to reactive plasmacytoses but less than multiple myeloma.
    • Significant differences in immunophenotype were observed between PLNPs and reactive plasmacytoses, other extramedullary plasmacytomas, and multiple myeloma.
    • PLNPs did not show progression to multiple myeloma, and survival rates were comparable to other extramedullary plasmacytomas.

    Conclusions:

    • Primary lymph node plasmacytoma exhibits distinct immunophenotypic characteristics.
    • PLNPs appear to be a separate entity from other plasma cell dyscrasias, including multiple myeloma and non-nodal extramedullary plasmacytomas.
