Ectopic parathyroid glands in the intravagal location are a rare cause of persistent hyperparathyroidism (HPT). This finding highlights the need to consider this unusual site during surgical evaluation for HPT.

Area of Science:

  • Endocrinology
  • Surgical Pathology

Background:

  • Ectopic parathyroid glands are a common cause of persistent hyperparathyroidism (HPT).
  • High cervical ectopic glands account for 0.21–9% of HPT cases.
  • Intravagal parathyroid glands are exceptionally rare, with only three prior cases reported.

Observation:

  • This study reports two new cases of persistent HPT attributed to an intravagal parathyroid gland.
  • These cases represent 0.12% of 1,712 operated HPT cases over 25 years.
  • One case involved primary HPT, the other tertiary HPT, with patients undergoing multiple surgeries.

Findings:

  • Intravagal parathyroid tissue can manifest as a cause of HPT.
  • The intravagal location is a rare ectopic site for parathyroid tissue.
  • Persistent or recurrent HPT may stem from such rare ectopic glands.

Related Experiment Videos

Implications:

  • The intravagal location should be considered in the differential diagnosis of high cervical ectopic parathyroid glands.
  • Preoperative and intraoperative assessments for hyperparathyroidism should include this rare site.
  • Recognition of this rare ectopic site is crucial for managing persistent or recurrent HPT.
