Unilateral adrenal histoplasmosis, a rare fungal infection in India, was diagnosed in a 56-year-old male. Treatment with amphotericin B led to a full recovery, highlighting the importance of considering this diagnosis in endemic regions.

Background:

  • Histoplasma capsulatum (HC) infection is uncommon in India.
  • Adrenal involvement in HC is exceptionally rare, especially presenting as a unilateral mass.

Observation:

  • A 56-year-old male presented with hepatosplenomegaly, unilateral adrenal mass, and significant weight loss.
  • Fine-needle aspiration cytology (FNAC) of the adrenal mass was inconclusive.
  • Histology of adrenalectomy specimen confirmed HC exclusively in the adrenal mass.

Findings:

  • The patient tested positive for histoplasmin.
  • Successful treatment was achieved with parenteral amphotericin B.
  • The patient showed complete recovery and remains asymptomatic under follow-up.
