Background:

  • Thyroid carcinoma is a significant health concern.
  • Early detection of thyroid cancer remains a challenge.

Purpose of the Study:

  • To investigate the potential of serum biomarkers for predicting thyroid carcinoma.
  • To analyze pre-diagnostic levels of thyroglobulin, TSH, and thyroxine in relation to thyroid cancer development.

Main Methods:

  • Retrospective analysis of serum samples from a biological bank.
  • Inclusion of 59 thyroid carcinoma cases (papillary and follicular) and 164 matched controls.
  • Measurement of serum human thyroglobulin (hTg), thyroid-stimulating hormone (TSH), and thyroxine (T4).

  • Significantly elevated serum thyroglobulin concentrations (≥30 μg/L) were observed in thyroid carcinoma cases compared to controls (OR 7.0).
  • Elevated thyroglobulin was present in 44% of carcinoma cases, with sensitivity around 50% for samples taken up to 15 years pre-diagnosis.
  • Specificity for elevated thyroglobulin was 89%, with no significant differences in TSH or T4 levels between cases and controls.

Conclusions:

  • Serum thyroglobulin elevation is a potential predictive biomarker for thyroid carcinoma.
  • The predictive value of serum thyroglobulin decreases with longer intervals prior to diagnosis.
  • TSH and thyroxine levels do not appear to be useful pre-diagnostic biomarkers for thyroid cancer in this cohort.
