Background:

Corticosteroids are widely used for inflammatory skin conditions.
Developing potent topical corticosteroids with reduced systemic side effects remains a key challenge.
Pyrazolo corticoids represent a novel class of synthetic steroids.

Purpose of the Study:

To synthesize and evaluate a series of novel substituted pyrazolo corticoids.
To identify compounds with a favorable separation of local anti-inflammatory activity from systemic effects.
To assess the clinical efficacy of promising candidates in treating psoriasis.

Main Methods:

Chemical synthesis of a range of pyrazolo corticoid derivatives.
In vivo animal models to assess the ratio of local to systemic activity.
Ex vivo vasoconstriction assays in human volunteers.
Clinical trials for psoriasis treatment.

The synthesis yielded several substituted pyrazolo corticoids.

Compound 21 demonstrated significant vasoconstriction activity in human volunteers.

Compound 21 displays a promising therapeutic profile for topical dermatological applications.
The enhanced local activity and reduced systemic exposure of compound 21 warrant its further development for psoriasis treatment.
