    Background:

    • Waldenstrom's macroglobulinaemia is a rare B-cell lymphoproliferative disorder characterized by a monoclonal IgM paraprotein.
    • The potential for monoclonal immunoglobulins to exhibit autoantibody activity is recognized.
    • Kidney involvement can occur in Waldenstrom's macroglobulinaemia, but specific autoantibody targets are not always identified.

    Observation:

    • Serum from a patient with Waldenstrom's macroglobulinaemia demonstrated intense immunofluorescent staining of kidney nuclei.
    • The monoclonal IgM protein and its Fab mu fragments were isolated and characterized.
    • In vitro studies confirmed the reactivity of the IgM and Fab fragments with kidney nuclei in unfixed cryostat sections of rodent kidneys.

    Findings:

    • The monoclonal IgM protein isolated from the patient's serum exhibited significant antinuclear activity.
    • This antinuclear activity was specifically directed against kidney nuclei.
    • The disappearance of the monoclonal IgM following chemotherapy correlated with the loss of detectable antinuclear activity in the serum.
