Summary

Pulmonary sarcoidosis can cause rare, severe hemoptysis, even early in disease. This case highlights systemic hypervascularization around granulomas as a cause, requiring surgery when embolization failed.

Area of Science:

  • Pulmonary Medicine
  • Cardiovascular Medicine
  • Oncology

Background:

  • Sarcoidosis is an inflammatory disease characterized by granuloma formation.
  • Hemoptysis, or coughing up blood, is an uncommon but serious complication of sarcoidosis.
  • It typically affects patients with advanced, fibrotic lung disease.

Observation:

  • A 36-year-old female with type II pulmonary sarcoidosis experienced severe hemoptysis early in her disease course.
  • Initial attempts at bronchial artery embolization were unsuccessful in controlling the bleeding.
  • Surgical intervention was ultimately necessary to manage the life-threatening hemorrhage.

Findings:

  • Clinical, microbiological, radiological, and pathological data were analyzed.
  • The hemoptysis was attributed to systemic hypervascularization surrounding sarcoidosis granulomas.

Related Experiment Videos

  • This finding suggests an unusual mechanism for bleeding in sarcoidosis.
  • Implications:

    • This case expands the understanding of hemoptysis causes in sarcoidosis.
    • It highlights the potential for early-onset, severe bleeding due to hypervascularization.
    • It underscores the importance of considering surgical options when less invasive treatments fail.
