Summary

Pediatric pheochromocytoma (PHEO) management requires early diagnosis and complete surgical excision. Incomplete removal and advanced disease significantly worsen outcomes in children with PHEO.

Area of Science:

  • Pediatric Endocrinology
  • Oncology
  • Surgical Oncology

Background:

  • Pediatric pheochromocytoma (PHEO) is rare, making its etiology, management, and outcomes poorly understood.
  • This study presents a comprehensive analysis of pediatric PHEO based on extensive single-center experience.

Purpose of the Study:

  • To elucidate the full spectrum of pediatric pheochromocytoma (PHEO) through a retrospective analysis of a large, single-center cohort.
  • To provide updated insights into the etiopathogenesis, management strategies, and outcomes of childhood PHEO.

Main Methods:

  • Retrospective review of medical records for pediatric patients diagnosed with PHEO between 1970 and 1999.
  • Data collection included patient demographics, medical history, clinical presentation, diagnostic methods, treatment interventions, pathological findings, and patient outcomes.

Main Results:

  • Sixteen children (12 boys, 4 girls) with a mean age of 10.7 years were treated for PHEO.
  • Hypertension was the most common symptom; 14 cases were sporadic, while 2 had associated genetic syndromes (VHL, MEN2b).
  • Complete excision was achieved in localized disease, but recurrence and malignancy indicators were noted, with a 19% mortality rate over 16 years.

Conclusions:

  • Early diagnosis and complete surgical excision are crucial for optimal outcomes in pediatric PHEO.
  • Multidisciplinary management, including pre- and post-operative medical therapy, is essential.
  • Incomplete excision and advanced disease stage are key predictors of poor prognosis; genetic evaluation is recommended for affected children.
