Background:

  • Topical anesthetics are used to reduce pain from local anesthetic injections.
  • EMLA cream (eutectic mixture of local anesthetics) and LET solution (lidocaine, epinephrine, tetracaine) are common options.

Purpose of the Study:

  • To compare the anesthetic efficacy of EMLA cream versus LET solution for pretreating lacerations before lidocaine injection.

Main Methods:

  • A randomized, double-blind trial involving 60 patients with traumatic lacerations.
  • Patients received either LET or EMLA applied to the laceration.
  • Anesthesia adequacy was assessed, and pain from subsequent lidocaine infiltration was recorded on a visual analog scale.

  • More wounds treated with LET achieved adequate anesthesia (73%) compared to EMLA (40%).
  • However, there was no significant difference in the reported pain of lidocaine infiltration between the two groups (median 12 mm for LET vs. 13 mm for EMLA).

  • Pretreatment with either LET solution or EMLA cream results in comparable pain levels during subsequent lidocaine infiltration for simple lacerations.
  • Both agents are effective in managing pain associated with local anesthetic administration in this setting.
