Background:

Total ankle arthroplasty (TAA) is increasingly recognized as a viable alternative to ankle arthrodesis (fusion).
Advancements in implant technology have led to innovative TAA designs.

Observation:

Modern TAA implants utilize syndesmosis fusion and resurfacing of ankle joint recesses or employ three-component, mobile bearing designs.
Limited clinical studies have evaluated the early outcomes of these contemporary prosthetic approaches.

Findings:

Early clinical results for both syndesmosis fusion/resurfacing and mobile bearing TAA designs are encouraging.
TAA demonstrates effectiveness in pain relief and functional improvement for carefully selected patients with end-stage ankle arthritis.

Implications:

Total ankle replacement can be offered as an alternative to arthrodesis for managing severe ankle arthritis.
Future research should focus on prosthetic designs that minimize bone resection and optimize restoration of natural ankle motion.
