A rare case of aortic root aneurysm developed six years after aortic valve replacement. Surgical intervention using the Bentall procedure was successful, highlighting the need for vigilance in patients with prosthetic valves.

Area of Science:

  • Cardiovascular Surgery
  • Aortic Surgery
  • Prosthetic Valve Surgery

Background:

  • A 65-year-old male with a history of aortic regurgitation underwent aortic valve replacement with a St. Jude Medical prosthetic valve.
  • Initial aortic root diameter was 40 mm, with a normal ascending aorta.
  • Follow-up revealed the development of a significant aortic root aneurysm.

Observation:

  • Aortic root aneurysm measured 60 mm in diameter, characterized as pear-like.
  • Diagnostic imaging included chest-enhanced computed tomography and chest aortography.
  • The patient underwent elective surgical repair using the Bentall procedure.

Findings:

  • Composite graft replacement with coronary reconstruction was performed using a 28-mm Hemashield graft and a 23-mm St. Jude Medical valve.

Related Experiment Videos

  • Coronary arteries were reconstructed with an 8-mm Hemashield graft for the left main and direct anastomosis for the right coronary artery.
  • Histological examination of the aneurysm wall revealed cystic medial necrosis.
  • Implications:

    • Aortic root aneurysm following aortic valve replacement is uncommon but carries a high risk of rupture or dissection.
    • Prompt surgical intervention is crucial for managing such rare but serious complications.
    • This case underscores the importance of long-term surveillance in patients with prosthetic valves and aortic root dilation.
