Background:

Liposarcomas represent 20% of soft tissue sarcomas.
Retroperitoneal liposarcomas account for 14% of liposarcomas, often originating from perirenal fat.

Observation:

A patient presented with symptoms of abdominal pain, distension, and weight gain.
Magnetic resonance imaging revealed a large retroperitoneal tumor, suspected to be a well-differentiated liposarcoma.

Findings:

The patient underwent successful curative resection of the massive retroperitoneal liposarcoma.
Complete tumor excision is critical, as only 50% of cases achieve this, with a 90% recurrence rate within 10 years.

Implications:

Curative resection remains the primary treatment for primary and recurrent liposarcomas.
