Summary

Symptomatic vascular rings are rare in adults. Surgical division of the left ligamentum arteriosum effectively treated dysphagia caused by a right-sided aortic arch in a 48-year-old woman.

Background:

  • Vascular rings, congenital anomalies of the aortic arch, are typically diagnosed in infancy.
  • Adult-onset symptomatic vascular rings are uncommon, often presenting with nonspecific symptoms like dysphagia.
  • Right-sided aortic arch with aberrant left subclavian artery and a left-sided ligamentum arteriosum can form a vascular ring compressing the esophagus.

Observation:

  • A 48-year-old woman presented with progressive dysphagia.
  • Diagnostic imaging revealed a right-sided aortic arch with an aberrant left subclavian artery and a left-sided ligamentum arteriosum.
  • The ligamentum arteriosum was compressing the esophagus, causing the dysphagia.

Findings:

  • The patient underwent a left posterolateral thoracotomy.
  • Division of the left ligamentum arteriosum was performed.
  • Postoperative recovery was uneventful, with complete resolution of dysphagia.

Implications:

  • This case highlights that vascular rings can present with adult-onset dysphagia lusoria.
  • Surgical division of the ligamentum arteriosum is an effective treatment for this specific vascular ring anatomy.
  • Left thoracotomy is a recommended surgical approach for this condition in adults.
