Background:

  • Sacral stress fractures are infrequently reported, particularly in the context of pregnancy and postpartum recovery.
  • Nontrauma-related fractures of the sacrum during the postpartum period are exceptionally rare.

Observation:

  • A 36-year-old woman presented with buttock pain four weeks after her first delivery.
  • Low Vitamin D levels were noted, but osteomalacia was absent. Standard laboratory tests and bone densitometry were unremarkable.

Findings:

  • A diagnosis of nontrauma-related postpartal sacral fracture was made.
  • This represents one of only five reported cases in the medical literature.

Implications:

  • Rheumatologists and orthopedic specialists should consider sacral fracture in postpartum patients experiencing buttock pain.
  • Magnetic resonance imaging (MRI) is recommended as the preferred diagnostic modality for evaluating suspected sacral fractures.
