Atypical squamous metaplastic (ASM) cells often indicate high-grade squamous intraepithelial lesions (HGSIL). While 62% of ASM cases showed SIL on biopsy, poor diagnostic reproducibility highlights the need for improved criteria and education.

- Refined diagnostic criteria, continuing education, and second opinions are recommended to improve accuracy.
