Summary

Second liver resection for recurrent hepatocellular carcinoma (HCC) is safe but offers low disease-free survival. Careful patient selection is crucial for repeat hepatectomy success in managing HCC recurrence.

Area of Science:

  • Hepatobiliary Surgery
  • Surgical Oncology
  • Gastroenterology

Background:

  • Hepatectomy is a standard treatment for primary hepatocellular carcinoma (HCC).
  • The efficacy of repeat liver resection for intrahepatic HCC recurrence after initial surgery remains under-evaluated.
  • Understanding outcomes of second liver resections is critical for managing recurrent HCC.

Purpose of the Study:

  • To evaluate the safety and efficacy of repeat liver resection for intrahepatic recurrence of HCC.
  • To compare outcomes of second hepatectomy with nonsurgical ablative therapy for recurrent HCC.
  • To identify factors influencing survival and disease-free survival after repeat liver resection.

Main Methods:

  • Retrospective review of 94 patients who underwent curative liver resection for HCC between 1991 and 1996.

Related Experiment Videos

  • Analysis of 57 patients with isolated liver recurrence: 12 underwent repeat resection, 45 received nonsurgical therapy.
  • Data collected on operative morbidity, mortality, survival, disease-free survival, and failure patterns.
  • Main Results:

    • Second liver resections for recurrent HCC had no perioperative deaths and comparable morbidity to initial resections.
    • Disease-free survival at 2 years was 31% after second hepatectomy, significantly lower than 62% after initial hepatectomy (p=0.009).
    • Overall survival at 2 years was 90% after second hepatectomy versus 70% after nonsurgical treatment (p=0.253).

    Conclusions:

    • Repeat liver resection for recurrent HCC can be performed safely and may improve overall survival.
    • The low disease-free survival rate after second hepatectomy highlights the high likelihood of further recurrences.
    • Further research is needed to select patients who will benefit most from repeat liver resection for HCC.
