Background:

Current diagnostic systems may not fully capture subthreshold and atypical psychiatric conditions.
There is a need for validated assessment instruments for these less-characterized conditions.
The Collaborative Spectrum Project addresses this gap by developing new diagnostic tools.

Purpose of the Study:

To develop and validate new instruments for assessing the spectrum of five psychiatric disorders.
To improve the characterization of subthreshold and atypical presentations of these disorders.
To provide reliable and valid tools for clinical and research use.

Main Methods:

Development of five structured clinical interviews: SCI-PAS (panic-agoraphobia), SCI-MOODS (mood), SCI-SHY (social phobia), SCI-OBS (obsessive-compulsive), and SCI-ABS (eating disorders).

Validation through multicenter studies in Italy and a cross-sectional study in Pittsburgh.

Administration alongside diagnostic interviews and other self-report/interviewer-rated instruments.

Main Results:

All interviews demonstrated high test-retest reliability (ICC > 0.61) and satisfactory internal consistency.
Scores were significantly higher in patient groups compared to controls.
Convergent validity was satisfactory for panic-agoraphobia, social phobia, and obsessive-compulsive spectra.

Conclusions:

The developed spectrum interviews possess highly satisfactory psychometric properties.
Ongoing studies aim to further establish the clinical validity of all spectra.
Subthreshold and atypical psychiatric symptoms warrant greater attention in epidemiological research.
