Background:

  • Stiff-man syndrome (SMS) is a rare neurological disorder.
  • Characterized by involuntary rigidity and continuous motor unit activity.
  • Autoantibodies to glutamic acid decarboxylase (GAD) are implicated in 60% of cases.

Observation:

  • This study reports on two patients with SMS negative for GAD autoantibodies.
  • Plasma exchange (PE) was used as a treatment modality.
  • Clinical outcomes were assessed post-treatment.

Findings:

  • One patient experienced minimal clinical improvement after PE.
  • The second patient reported subjective improvement, comparable to medication effects.
  • Results suggest autoantibody-negative SMS patients may respond less favorably to PE.

Implications:

  • Plasma exchange may not be an effective treatment for Stiff-man syndrome patients without GAD autoantibodies.
  • Further research is needed to explore alternative or aggressive PE strategies.
  • Understanding autoantibody status is crucial for guiding SMS treatment decisions.
