Background:

  • Tuberculous pericarditis is a significant cause of pericardial effusion, particularly in endemic regions.
  • Constrictive pericarditis can arise as a sequela of inflammatory pericardial conditions, including tuberculosis.

Observation:

  • A 75-year-old male presented with anorexia and was diagnosed with tuberculous pleuritis and pericarditis based on elevated adenosine deaminase (ADA) in pericardial effusion and positive tubercle bacilli culture.
  • Despite chemotherapy, the patient developed recurrent massive pleural effusion requiring drainage and subsequently showed signs of constrictive pericarditis, including pericardial thickening on CT and impaired cardiac function on catheterization.

Findings:

  • The patient was diagnosed with constrictive pericarditis secondary to tuberculous pericarditis.
