Area of Science:

  • Neurosurgery
  • Neurology
  • Pain Management

Context:

  • Recurrent trigeminal neuralgia (TN) after initial microvascular decompression (MVD) presents a clinical challenge.
  • Understanding the causes of TN recurrence post-MVD is crucial for effective retreatment.

Purpose:

  • To investigate the causes of recurrent trigeminal neuralgia following microvascular decompression.
  • To evaluate the outcomes of repeat surgical interventions for recurrent TN.

Summary:

  • Twelve patients with recurrent TN post-MVD underwent repeat operations.
  • Vascular loop recompression at the trigeminal root was identified in 10 patients; 2 had negative explorations.
  • All patients achieved pain relief through neurovascular redecompression or neurolysis, with no recurrence during 6 months to 4 years follow-up.

Related Experiment Videos

Impact:

  • Repeat surgical intervention, including redecompression or neurolysis, is effective for managing recurrent trigeminal neuralgia.
  • Identifying vascular compression as a primary cause of recurrence guides surgical decision-making for repeat operations.
