Summary

Women with gestational diabetes have higher plasma leptin levels during pregnancy and postpartum, associated with insulin resistance and weight gain. Leptin levels decrease after delivery but remain elevated compared to non-pregnant individuals.

Area of Science:

  • Endocrinology
  • Metabolic Disorders
  • Reproductive Health

Background:

  • Pregnancy is characterized by insulin resistance, altered body weight, and energy metabolism.
  • The impact of pregnancy on plasma leptin concentrations, particularly in gestational diabetes, is not fully understood.

Purpose of the Study:

  • To determine plasma leptin levels in women with gestational diabetes compared to normal pregnancy and non-pregnant controls.
  • To investigate the relationship between plasma leptin, glucose metabolism, insulin resistance, and body weight changes during pregnancy.

Main Methods:

  • Longitudinal study measuring plasma leptin and B-cell hormones at 28 gestational weeks.
  • Measurements taken fasting and after an oral glucose tolerance test (OGTT) in women with gestational diabetes, normal glucose tolerance, and non-pregnant controls.

Related Experiment Videos

Main Results:

  • Plasma leptin was significantly higher in women with gestational diabetes (24.9 ng/ml) and normal glucose tolerance (18.2 ng/ml) compared to non-pregnant controls (8.2 ng/ml).
  • OGTT did not alter plasma leptin. Gestational diabetes was associated with higher basal insulin, marked insulin resistance, and correlations between leptin, glucose, insulin, BMI, and weight gain.
  • Postpartum, leptin decreased but remained higher than in non-pregnant women.

Conclusions:

  • Women with gestational diabetes exhibit elevated plasma leptin during and after pregnancy, independent of oral glucose loading.
  • Plasma leptin is strongly associated with glucose, insulin resistance, and body weight changes in gestational diabetes.
  • Leptin levels do not fully normalize postpartum in women with gestational diabetes, suggesting persistent metabolic alterations.
