Linear and whorled nevoid hypermelanosis (LWNHM) is a rare genetic mosaicism disorder. This case highlights LWNHM presentation and normal neurological findings in a 15-year-old girl.

Background:

  • Linear and whorled nevoid hypermelanosis (LWNHM) is a rare genodermatosis characterized by hyperpigmented macules following Blaschko's lines.
  • It is often sporadic, suggesting genetic mosaicism, and may be associated with neurological abnormalities.

Observation:

  • A 15-year-old girl presented with progressively increasing streaks of reticulate hyperpigmented macules in a whorled pattern on her trunk and extremities since birth.
  • No preceding eruption or systemic abnormalities were reported.
  • Histopathology showed basal cell hyperpigmentation without pigmentary incontinence.

Findings:

  • The patient's LWNHM presentation was consistent with the described reticulate pattern along Blaschko's lines.
  • Neurological assessment, including a CT scan, revealed no abnormalities.
  • The disorder resembled incontinentia pigmenti and epidermal nevus clinically.

Implications:

  • This case contributes to the understanding of LWNHM's clinical spectrum and presentation.
  • It underscores the importance of differentiating LWNHM from similar pigmentary disorders.
  • The absence of neurological abnormalities in this case broadens the known clinical variability of LWNHM.
