Background:

  • The mitral apparatus is crucial for global left ventricular (LV) function.
  • Previous studies suggest chordal transection unloads myocardium at the papillary muscle insertion site.

Purpose of the Study:

  • To investigate if the regional response to afterload differs between the papillary muscle insertion site and the LV free wall.
  • To test the hypothesis that chordal tension alters the papillary muscle site's response to afterload.

Main Methods:

  • Studied end-systolic pressure-regional segment length relations (ESPLR) in anterior LV free wall (FREE) and papillary muscle insertion (PAP) sites.
  • Used sonomicrometer crystals and echocardiography in 7 anesthetized open-chest dogs.
  • Increased afterload via aortic occlusion.

  • ESPLR were highly linear at both FREE and PAP sites (r >= 0.9).
