Summary

Large hyperplastic polyps, though rare, are unexpected colon findings. These polyps require endoscopic removal and ongoing patient surveillance for potential metachronous lesions.

Area of Science:

  • Gastroenterology
  • Colorectal Surgery
  • Pathology

Background:

  • Hyperplastic polyps are common, small, non-neoplastic colon lesions, typically found in the rectosigmoid area.
  • Recent research suggests hyperplastic polyps may serve as markers for neoplastic polyps.
  • Standard hyperplastic polyps are usually sessile and less than 5 mm in size.

Observation:

  • This report details four uncommon cases of large (20 mm) hyperplastic polyps.
  • These large polyps were identified at a single institution over a nine-year period.
  • Locations varied, with two in the right colon, and one was pedunculated.

Findings:

  • All four large hyperplastic polyps were successfully removed via outpatient endoscopic polypectomy without complications.
  • None of the cases were associated with synchronous neoplastic polyps or polyposis syndromes.

Related Experiment Videos

  • Follow-up colonoscopies in three patients revealed no evidence of metachronous polyps.
  • Implications:

    • Large hyperplastic polyps are rare, challenging to differentiate from neoplastic lesions, and not site-specific.
    • Endoscopic removal using standard techniques is recommended for these large polyps.
    • Long-term endoscopic surveillance is crucial for patients with large hyperplastic polyps to monitor for new polyp development.
