Summary

Posterior subtenon anesthesia can cause globe perforation, especially in patients with prior eye surgery. Surgeons should use extra caution or consider alternative anesthesia methods to prevent this complication.

Background:

  • Posterior subtenon anesthesia is a common technique for ophthalmic procedures.
  • Complications, though rare, can occur during its administration.

Observation:

  • A case report details a globe perforation during the initiation of posterior subtenon anesthesia.
  • The perforation occurred while dissecting beneath the Tenon capsule with scissors.

Findings:

  • The patient had a history of retinal detachment in both eyes.
  • The complication arose during the anesthetic procedure for a second retinal detachment repair in the left eye.

Implications:

  • Increased caution is necessary when administering sub-Tenon anesthesia in patients with a history of ophthalmic surgery, thinned sclera, or scar tissue.
