Background:

  • Isolated subaortic stenosis (ISAS) is a rare congenital cardiac anomaly with discrete and tunnel types.
  • Discrete ISAS involves a fibrous obstruction below the aortic valve, potentially causing significant left ventricular outflow tract obstruction.

Observation:

  • A 22-year-old female presented with exertional dyspnea and a murmur.
  • Echocardiography revealed severe aortic stenosis, moderate aortic insufficiency, left ventricular hypertrophy, and membranous subaortic stenosis with a 121 mmHg gradient.

Findings:

  • Surgical aortic valve replacement and excision of the membranous subaortic stenosis collar were performed.
  • The patient experienced a successful postoperative recovery.
