A rare frontotemporal dermoid cyst presented with multiple discharging sinuses and unusual intraorbital and intracranial extensions in a 14-month-old girl. Surgical excision was successfully performed, addressing all components of this complex congenital anomaly.

Area of Science:

  • Neurosurgery
  • Pediatric Surgery
  • Ophthalmology

Background:

  • Congenital dermoid cysts are rare tumors that can occur in various locations, including the head and neck.
  • Frontotemporal dermoid cysts with intracranial or intraorbital extensions are exceptionally uncommon.
  • Discharging sinuses associated with these cysts are seldom reported.

Observation:

  • A 14-month-old female presented with a complex frontotemporal dermoid cyst.
  • The cyst exhibited three discharging sinuses over the temporal area, lower eyelid, and cheek.
  • Radiological imaging revealed significant intraorbital and intracranial extensions through the temporal bone.

Findings:

  • The presented case is a rare instance of a frontotemporal dermoid cyst with multiple discharging sinuses and extensive intracranial and intraorbital involvement.

Related Experiment Videos

  • Preoperative computed tomography (CT) guided the surgical planning, especially given the history of orbital cellulitis.
  • Surgical excision was achieved via a hemicoronal-preauricular incision, necessitating a lateral orbitotomy for complete removal of the intraorbital component.
  • Implications:

    • This case highlights the importance of considering rare presentations of congenital cysts, even with seemingly straightforward initial symptoms.
    • Comprehensive imaging is crucial for delineating the full extent of complex cysts, particularly those with intracranial and intraorbital extensions.
    • Successful surgical management demonstrates the feasibility of excising extensive dermoid cysts, improving patient outcomes and preventing further complications.
