Popliteal Artery Entrapment Syndrome (PAES) is a rare congenital condition causing leg ischemia in young males. Surgical treatment involving thrombus removal and artery repair led to a full recovery in a 14-year-old patient.

Background:

  • Popliteal Artery Entrapment Syndrome (PAES) is a rare congenital vascular anomaly.

Observation:

  • A 14-year-old male presented with acute 24-hour right leg ischemia.
  • The ischemia was diagnosed as secondary to Popliteal Artery Entrapment Syndrome.
  • A specific anatomical variation involving the gastrocnemius muscle was identified.

Findings:

  • A posterior surgical approach to the popliteal artery was utilized.
  • Division of an accessory slip of the gastrocnemius muscle relieved the arterial compression.
  • An arteriotomy allowed for the removal of a floating thrombus.
  • Direct continuous suture reconstruction of the popliteal artery was performed.

Implications:

  • Successful surgical management of PAES can restore arterial patency and prevent limb loss.
  • This case highlights the importance of considering PAES in young patients with acute leg ischemia.
  • Prompt diagnosis and surgical intervention lead to excellent long-term outcomes, as evidenced by the patient's symptom-free status and palpable pulse one year post-surgery.
