Insights
Maintaining good health in older adults is linked to lifestyle choices like physical activity and not smoking. These factors, along with managing cardiovascular risks, are key to healthy aging.
Area of Science:
- Gerontology
- Preventive Medicine
- Public Health
Background:
- Identifying factors for healthy aging is crucial for public health initiatives.
- Older adults represent a growing demographic with unique health challenges.
- Understanding predictors of health maintenance can inform interventions.
Purpose of the Study:
- To determine factors associated with remaining healthy in adults aged 65 and older.
- To explore the role of behavioral and cardiovascular risk factors in health maintenance.
Main Methods:
- Longitudinal cohort study of 5,888 participants aged 65+ from the Cardiovascular Health Study.
- Assessment of chronic disease at baseline and monitoring for incident disease and mortality over 7 years.
- Analysis of behavioral factors (physical activity, smoking) and cardiovascular risk factors (HDL cholesterol, blood pressure, diabetes).
Main Results:
- A significant proportion of older adults remained healthy throughout the follow-up period.
- Physical activity, not smoking, and moderate wine consumption (women) were associated with sustained health.
- Higher HDL cholesterol, lower blood pressure, and absence of diabetes were linked to better health outcomes.
- Exercise, smoking cessation, and aspirin use remained significant health predictors after controlling for other factors.
Conclusions:
- Modifiable behavioral factors like physical activity and smoking cessation are vital for maintaining health in older adults.
- Management of cardiovascular risk factors, including diabetes, HDL cholesterol, and blood pressure, is essential for healthy aging.
- These findings support the development of lifestyle interventions to promote health in the elderly population.
Objectives:
To identify factors associated with remaining healthy in older adults.
Design:
Longitudinal cohort study.
Setting:
Data were collected at the four Cardiovascular Health Study field centers.
Participants:
5,888 participants age 65 years and older in the Cardiovascular Health Study.
Measurements:
Presence of chronic disease was assessed at baseline and over a maximum 7-year follow-up period. Participants who were free of chronic disease (no cardiovascular disease (CVD), chronic obstructive pulmonary disease, or self-reported cancer, except nonmelanoma skin cancer) at the baseline examination were then monitored for the onset of incident cancer, cardiovascular disease, and fatal outcomes.
Results:
A high proportion of these older adults was healthy at the initial examination and remained healthy over the follow-up period. Numerous behavioral factors were associated with continued health, including physical activity, refraining from cigarette smoking, wine consumption (women), higher educational status, and lower waist circumference. A number of CVD risk factors and subclinical disease measures were associated with continued health, including higher high-density lipoprotein (HDL) cholesterol, lack of diabetes, thinner common carotid intimal nmedial thickness, lower blood pressure, lower C-reactive protein, and higher ankle-arm blood pressure ratio. Among the behavioral factors, exercise, not smoking, and not taking aspirin remained significant predictors of health even after controlling for CVD risk factors and subclinical disease in older adults.
Conclusions:
These data suggest that a number of modifiable behavioral factors (physical activity, smoking, and obesity) and cardiovascular risk factors (diabetes, HDL cholesterol, and blood pressure) are associated with maintenance of good health in older adults.
