Background:

  • Acute urinary retention (AUR) in men often leads to prostatectomy, increasing risks of complications and hospital death.
  • Indwelling urethral catheterization for AUR carries a daily risk of bacterial colonization.
  • A trial without catheter (TWOC) is preferred to potentially avoid surgery or catheter dependence.

Purpose of the Study:

  • To evaluate the efficacy of alpha(1) blockers in improving TWOC success rates for AUR.
  • To assess the impact of alfuzosin on TWOC outcomes and identify predictors of recurrence.

Main Methods:

  • A placebo-controlled study involving 81 patients with AUR undergoing a TWOC.
  • Patients were randomized to receive either alfuzosin or a placebo.
  • Long-term follow-up was conducted to monitor for recurrent AUR episodes and need for surgery.

Main Results:

  • Successful TWOC was achieved in 55% of patients treated with alfuzosin, compared to 29% in the placebo group (p=0.03).
  • 32% of patients experienced a subsequent AUR episode within a mean of 4.1 months.

Conclusions:

  • Alfuzosin significantly improves the success rate of TWOC in men with AUR.
  • Identifying patients with high PVR post-TWOC allows for targeted follow-up and early intervention.
  • Alfuzosin's PVR-reducing effect may help prevent recurrent urinary retention after TWOC.
