Background:

  • Telemedicine (TM) allows remote specialist evaluations via closed-circuit television.
  • The applicability of TM for vascular surgery patients, requiring physical pulse palpation, was questioned.

Purpose of the Study:

  • To test if TM is as effective as direct patient examination for developing vascular patient care plans.

Main Methods:

  • Sixty-four vascular evaluations were performed on 32 patients in rural outreach clinics.
  • Two vascular surgeons evaluated each patient: one on-site, one via TM, blinded to each other's findings.
  • A nonphysician assistant aided the TM physician with vital signs, Doppler, and equipment operation.

  • Physician concordance on treatment recommendations was 91% (29/32 patients).
  • Average TM and on-site evaluation durations were similar (20.6 vs. 19.0 minutes, P=not significant).
  • Physician confidence in TM history taking was 97% excellent; physical exam confidence was 70% excellent. All patients preferred local TM over travel.

Conclusions:

  • TM evaluation is accurate and effective for various vascular conditions, comparable to on-site assessments.
  • Physician experience and a knowledgeable on-site assistant are crucial for successful TM vascular evaluations.
  • TM technology can be readily adapted to other clinical specialties for remote patient care.
