Hormonal Regulation of the Menstrual Cycle

The ovarian cycle regulates endometrial changes throughout a single menstrual cycle via the coordinated action of gonadotrophin-releasing hormone (GnRH) and gonadotrophins.
At puberty, GnRH begins a pulsatile release pattern, which triggers the anterior pituitary gland to secrete follicle-stimulating hormone (FSH) and luteinizing hormone (LH). The frequency and amplitude of GnRH pulses vary across the menstrual cycle, with faster pulses favoring LH release and slower pulses favoring FSH release.

Background:

  • Estrogen may benefit postmenopausal women's cognition.
  • Raloxifene, a selective estrogen-receptor modulator, is investigated for cognitive effects.

Purpose of the Study:

  • To determine if raloxifene influences cognitive function in postmenopausal women with osteoporosis.

Main Methods:

  • 7478 postmenopausal women with osteoporosis were randomized to raloxifene (60 or 120 mg) or placebo for three years.
  • Cognitive function was assessed using six tests at baseline and various intervals up to three years.
  • Cognitive decline was defined as a score change in the worst 10 percent.

Main Results:

  • Cognitive scores showed slight improvement across all groups, with no significant differences.
  • Raloxifene did not significantly alter the risk of cognitive decline on most tests.
  • A trend towards reduced decline in verbal memory and attention was observed with raloxifene.

Conclusions:

  • Three-year raloxifene treatment did not affect overall cognitive scores in postmenopausal women with osteoporosis.
