Summary

Nasopharyngeal angiofibroma management primarily involves surgery. Preoperative embolization may be considered, though its impact on blood loss was not statistically significant in this study of 11 patients. No recurrences were observed during follow-up.

Area of Science:

  • Otorhinolaryngology
  • Surgical Oncology
  • Head and Neck Surgery

Background:

  • Nasopharyngeal angiofibromas are benign, locally invasive tumors affecting the nose and nasopharynx.
  • Surgery is the primary treatment modality for these neoplasms.
  • This study details the management experience with nasopharyngeal angiofibroma.

Purpose of the Study:

  • To present the clinical experience in managing nasopharyngeal angiofibroma.
  • To analyze the effectiveness of preoperative embolization on intraoperative blood loss.
  • To evaluate surgical outcomes and recurrence rates.

Main Methods:

  • Retrospective review of 13 nasopharyngeal angiofibroma cases from 1979-1999.
  • Eleven patients who underwent primary surgery were included for analysis.
  • Investigated the relationship between preoperative embolization and estimated blood loss (EBL).

Main Results:

  • The incidence was 1 per 5,000 ENT admissions, with a male predominance (10:1) and median onset age of 18.
  • Transpalatal approach was the most common surgical procedure (8/11 patients).
  • Preoperative embolization did not significantly reduce intraoperative EBL (814.3 ml vs. 587.5 ml); no recurrences were noted during an average 118.4-month follow-up.

Conclusions:

  • Surgery remains the cornerstone of nasopharyngeal angiofibroma treatment.
  • Preoperative embolization is suggested, despite non-significant impact on EBL in this series.
  • Radiotherapy serves as an adjuvant for advanced or recurrent cases.
