Background:

  • A 67-year-old female presented with headache and osteolytic lesions in the frontal bone.
  • MR imaging revealed mixed-intensity lesions with signal characteristics similar to gray matter and cerebrospinal fluid.
  • Angiography showed abnormal vascular staining from the middle meningeal artery.

Findings:

  • Surgical exploration identified numerous white granular masses penetrating the dura and adhering to the arachnoid membrane.
  • Histological examination confirmed these masses as normal arachnoid granulations and villi.
  • The case was diagnosed as ectopic arachnoid granulomatosis, a condition not previously reported away from venous sinuses.
