Ectopic arachnoid granulomatosis: a case report.

Background:

Arachnoid granulation, typically found near venous sinuses, can undergo hypertrophy and mimic tumors.
Differential diagnosis is essential when hypertrophied arachnoid granulation presents outside its usual location.
Cerebrospinal fluid absorption in abnormal angiographic stain regions was hypothesized.

Observation:

A 67-year-old female presented with headache and osteolytic lesions in the frontal bone.
MR imaging revealed mixed-intensity lesions with signal characteristics similar to gray matter and cerebrospinal fluid.
Angiography showed abnormal vascular staining from the middle meningeal artery.

Findings:

Surgical exploration identified numerous white granular masses penetrating the dura and adhering to the arachnoid membrane.
Histological examination confirmed these masses as normal arachnoid granulations and villi.
The case was diagnosed as ectopic arachnoid granulomatosis, a condition not previously reported away from venous sinuses.

Implications:

This case highlights the importance of considering ectopic arachnoid granulomatosis in the differential diagnosis of intracranial masses.
Understanding the presentation of arachnoid granulations outside venous sinuses is critical for accurate diagnosis and treatment.
Further research may elucidate the mechanisms behind ectopic arachnoid granulation formation and its clinical significance.
