Summary

Excessive postobstructive diuresis, a rare condition following unilateral ureteral obstruction, can lead to extreme fluid loss. This case highlights massive urine output from both kidneys after relieving a single ureteral blockage.

Area of Science:

  • Nephrology
  • Urology
  • Endocrinology

Background:

  • Postobstructive diuresis typically occurs after relief of bilateral or subvesical urinary obstruction.
  • Polyuria following unilateral obstruction is uncommon and less documented in medical literature.

Observation:

  • A 56-year-old female experienced extreme polyuria exceeding 58,000 ml daily after release of a left ureteral obstruction.
  • This excessive urine output originated from both the obstructed and the contralateral, unobstructed kidney.

Findings:

  • The case presents the highest reported daily urine output in postobstructive diuresis literature.
  • The pathomechanism involves complex, not fully elucidated kidney responses to the release of unilateral obstruction.

Implications:

Related Experiment Videos

  • This case underscores the potential for severe diuresis even after unilateral obstruction relief.
  • Management requires vigilant fluid and electrolyte replacement, with observed complete kidney function recovery.
