Summary

Symptomatic hypersensitivity pneumonitis can occur despite normal high resolution computed tomography (CT) scans. Open lung biopsy confirmed the diagnosis in a patient with systemic lupus erythematosus, leading to improvement after antigen removal.

Area of Science:

  • Pulmonology
  • Radiology
  • Immunology

Background:

  • Hypersensitivity pneumonitis (HP) is an immune-mediated lung disease.
  • High resolution computed tomography (HRCT) is a key diagnostic tool for HP.
  • Diagnostic challenges arise when HRCT findings are normal.

Observation:

  • A 32-year-old male with systemic lupus erythematosus presented with chronic respiratory symptoms.
  • Pulmonary function tests were abnormal, but HRCT scans of the chest were normal.
  • Diagnosis of HP was established via open lung biopsy.

Findings:

  • Symptomatic HP can present with normal HRCT findings.
  • Open lung biopsy is crucial for diagnosis in atypical cases.
  • Antigen avoidance led to clinical improvement.

Related Experiment Videos

Implications:

  • This case highlights the limitations of HRCT in diagnosing HP.
  • Clinicians should consider HP even with normal imaging, especially in immunocompromised patients.
  • Further research is needed on the role of imaging in HP diagnosis.
